Lake Taupō Hospice welcomes referrals for people who live in the greater Lake Taupō district (Including Tūrangi and Mangakino)
Hospice care is free to anyone who needs specialist support at any stage of a serious illness which has little or no chance of a cure.
We care for you, wherever you call home, working alongside your GP, other community and specialist teams to ensure the highest level of personalised care.
Together we’re committed to supporting your wellbeing medically, spiritually, culturally, psychologically and socially with an emphasis on helping people live every moment in whatever way is important to them.
WE'RE HERE FOR YOU DAYS, WEEKS OR MONTHS BEFORE DEATH
There is a common misconception that hospice is where you go at the very end of your illness.
While we do see people weeks or sometimes days before death, our priority is to help you live as fully and as well as you can with most of our care taking place out in the community wherever people call ‘home’.
The earlier we see you, the more we can help you and those who are caring for you.
HOW CAN I ACCESS HOSPICE CARE?
The most common way to be referred is through a healthcare professional such as your GP or medical specialist. You can also refer a family member, loved one (with their consent), or yourself. Hospice care is free of charge thanks to the generosity of our community.
I WANT TO REFER MYSELF OR A FAMILY MEMBER
We welcome self referrals or by a family member/loved one, with the consent of the patient. Please click here to complete our self referral form online. One of our staff will be in touch with you after they have reviewed your referral.
I’M A HEALTHCARE PROFESSIONAL
If you who would like to refer a patient to us, please click here to download and fill in the referral form. Please email file or scanned copy with any supporting documents to info@laketaupohospice.co.nz when completed.
Referrals can now be made directly from most GP practices via BPAC. This is a fast and easy way to refer and allows you to attach the most up to date patient notes and results.
